PhysioCare at Home offers one-on-one home visits to seniors, allowing us to devote extended time and exclusive attention to each client. With over 10 years of experience, we understand the unique partnership between physiotherapists and physicians in helping seniors to recover from surgeries, falls, and specific health conditions.
When you refer a client to us, you can expect trustworthy and consistent care while keeping you updated on the client’s progress through continued communication and reports. We offer you peace of mind, knowing your client is receiving quality, one-to-one care that is moving them toward their lifestyle goals.
Our physiotherapists create individualized home exercise programs to help the client work on mobility, reduce the risk of falls, control pain, and improve strength, flexibility, and stamina. In addition to making equipment recommendations, we also seek to include any family or caregivers in our care by providing them with relevant education and open communication.
Our team of physiotherapists use outcome measures to help our clients see their progress. These outcome measures are recorded and sent in progress reports to the clients’ physicians. Some of the commonly used outcome measures we use are:
- ABC (Activities-specific Balance Confidence Scale)
- Berg Balance
- DASH (Disabilities of Arm Shoulder and Hand)
- Dynamic Gait Index
- Elderly Mobility Scale
- Functional Reach Test
- Gait Speed Norms
- Sit to Stand
- TUG (Timed Up and Go Test)
You can help us by sending copies of any relevant imaging findings or specialist reports that may help better inform our physiotherapists in providing excellent care (our clients will sign consent forms authorizing the release of this information). Please note that the private in-home physiotherapy service we offer is often covered by extended health care plans as well as Veteran’s Affairs. We send updates on the clients’ statuses to their doctors over the duration of our care as well as upon discharge.
